Where can I find the Designated (Surrogate) Borrower Authorization Form?

A faculty member may authorize a designated borrower.  Designated borrowers may have some faculty borrowing privileges, including faculty delivery. For further information, please contact Central Library at 615-322-2800 or any other library that you wish the surrogate to be able to use.
